Pathobiological Effects of Hemolysis1

A direct result of hemolysis of RBCs is the release of hemoglobin into the plasma. This leads to circulating free hemoglobin, resulting in heme-scavenging mechanisms including nitric oxide scavenging and a decrease in nitric oxide synthesis.1

The resultant nitric oxide depletion causes1:

  • Endothelial cell dysfunction
  • Impaired regulation of smooth muscle tone
  • Platelet activation and aggregation
  • Local vasoconstriction

These pathobiological effects can lead to pulmonary and systemic hypertension, CKD, erectile dysfunction, dysphagia, abdominal pain, and intravascular thrombosis, all of which are sequelae of PNH.1
