High-Sensitivity Labs for PNH Diagnostic Laboratory Services

Flow cytometry, performed on peripheral blood, is the most sensitive and informative assay for PNH diagnosis.

Clear reporting is critical for appropriate clinical decisions.

When ordering a PNH flow test, request1:

  • Clone size for each cell lineage (ie, granulocytes, monocytes, and erythrocytes)
  • Proportion of type I, II, and III cells in cases in which PNH clones are detected
  • Sensitivity level used (0.01% sensitivity is ideal)
  • All previous flow results in order to monitor clonal expansion

With PNH, the use of high-quality pathology services that emphasize best practices and employ staff experienced in hematopathology are encouraged.

Labs should perform PNH flow cytometry, obtained from peripheral blood, on granulocytes and at least 1 additional cell line using >1 reagent.1,2

  • PNH consensus guidelines recommend routine (1% PNH cell threshold) or high-sensitivity (0.01% PNH cell threshold) flow cytometry testing on patients at higher risk for PNH1
  • Granulocytes should be evaluated with CD16, CD24, CD66b, or the FLAER assay1
  • Flow cytometry is noninvasive and relatively inexpensive
